NETWORK PROVIDER APPLICATION
Submission of an application does not guarantee acceptance. All applications are reviewed on a rolling basis. Please read all information below, prior to submitting your application.
DESCRIPTION: Licensed Clinical Professionals
The After Project™ Network Providers form the clinical foundation of our coordinated recovery model. Credential verification is required. Applicants must demonstrate trauma-informed experience, maintain active availability for new patients, and participate in collaborative, interdisciplinary care.
Network Providers must be willing to coordinate with other approved Network Providers, Adjunct Practitioners, and Animal-Supported Recovery Partners when clinically appropriate. This includes openness to integrative modalities and, within professional scope of practice, supporting members who utilize trained service animals.
Eligibility & Participation Requirements
Applicants must:
• Hold an active, unrestricted professional license in good standing
• Maintain current professional liability insurance
• Demonstrate trauma-informed training or documented experience
• Be actively accepting new patients and/or clients
• Maintain an average wait time of approximately two weeks or less for initial consultation while listed in the Network
• Support a coordinated recovery model that may include adjunctive therapies and animal-assisted recovery pathways
• Collaborate with interdisciplinary providers when clinically appropriate
• Notify The After Project™ of any lapse in licensure, insurance, or disciplinary action
What Happens After Approval
Approved Network Providers are considered for inclusion within The After Project™ coordinated referral network based on specialty, geographic availability, and alignment with the Foundation’s clinical standards.
If approved:
• Providers will be invited to execute a formal Participation Agreement outlining scope, expectations, confidentiality standards, and coordinated care guidelines.
• Licensure and credentials will be independently verified prior to activation.
• Providers may receive referral consideration for members aligned with their specialty and availability.
• Providers will participate in interdisciplinary collaboration when clinically appropriate.
• Periodic availability confirmation may be required to maintain active status.
• Providers will submit professional profile information for website listing (e.g., biography, specialties, service format). All website content will be reviewed and approved by the provider prior to publication.
Network Providers remain independent practitioners. The After Project™ does not employ providers, control clinical decision-making, or determine provider fees.
Providers are solely responsible for billing members or insurers through their own established systems.
The After Project™ reserves the right to revise participation standards as the Foundation expands its national infrastructure.
Submission of an application indicates understanding of and willingness to operate within this structured model.

All information submitted through this application is confidential and used solely for credential verification and candidate evaluation purposes.
